A. Duties/Responsibilities shall include, but are not limited to: 1. Pharmacist a. Prepare, manufacture, compound, and dispense drugs and pharmaceutical preparations; b. Provide computer order entry and type prescription labels; c. Fill prescription/medication orders and prepare to check I.V. additives as required; d. Provide consultations to medical staff and the Medical Review Committee and provide discharge consultation where required; e. Maintain records of drugs, poisons and narcotics as required by Federal and State laws; f. Maintain records of drugs, poisons and chemicals received, on hand and dispensed; g. Implement and/or maintain medication delivery systems as required including automated systems such as Talyst; h. Review prescription/medication order(s) against Resident’s chart and medication profile for improper drug selection, under-dosage, dosage, over-dosage, drug interactions, and dosage regimen; i. Assist in the preparation of estimates for the requisition of drugs and chemicals as required; j. Monitor and instruct non-professional assistants in standard and routine phases of the work, as appropriate; k. Assist with in-service training as needed; and l. Perform other duties as required and permitted within the scope of practice. B. Provider shall wear an identification badge which includes the name of the Contractor, the Provider’s name, and a photo ID. This badge shall be worn at all times while on grounds of the Veteran’s Home. C. Provider shall ensure resident medical record information is maintained as “CONFIDENTIAL” and shall be held in trust and confidence. Information shall be used only for the purposes contemplated under this contract.
