It is the intent of these specifications, terms, and conditions for Alameda County Health, Behavioral Health Department (hereafter ACBHD or County) to seek proposals to provide InHome Outreach Team (IHOT) services. The priority population for this program is adults ages 18 and up residing in Alameda County with Substance Use Disorder (SUD) with or without cooccurring mental health challenges, who are not currently engaged in substance use or community support services. Program services and supports shall include but not be limited to referrals, outreach, engagement, linkages via warm hand-off, and follow up. The IHOT program model is a short-term intervention designed to reach populations that do not engage in ongoing substance use services or treatment.
