Specifications include, but are not limited to: Marin County HHS Division of Whole Person Care is seeking collaborative proposals that show a strong inter-agency partnership to develop a robust program to provide reliable, regular, and relevant medical care and/or behavioral health services in the field to people experiencing homelessness. Applicants will be required to partner with another organization to create a proposal that does not lead to duplicative or fragmented services. The proposed programs should strive to achieve the following goals: - Providing high quality behavioral health and medical care with dignity - Filling gaps in care for people experiencing homelessness - Building trust with people experiencing homelessness through services that are reliable, regular, and relevant - Meeting people where they are at, literally and figuratively - Increasing preventive care - Decreasing the rate of accidental overdoses - Providing connection to a Primary Care medical home and other resources
