Specifications include, but are not limited to: The SCOPE of the requested service for the SHC evaluation fall into three categories: Project Management, Data Collection and Analysis, and Data Management. The following tasks are to be completed under the term of the awarded contract. Project Management: Ensure communication and coordination of all SHC evaluation stakeholders. • Develop Annual Project Management Plan • Participate in regular meetings with CHSC (monthly telephone/video meetings and yearly all-day retreat) to discuss, plan, problem solve around the SHC evaluation • Participate in SHC Lead Agency monthly meetings (Lead Agencies are the 10 organizations that operate the School Health Centers) • Refine the existing logic model to describe the performance measures and indicators • Prepare, submit, and get approval for protocols for conducting human research, as applicable • Provide technical assistance to align SHC evaluation with CHSC’s Strategic Plan around RBA • Collaborate with CHSC external evaluators, as necessary Data Collection and Analysis: Implement a comprehensive data collection approach at SHC sites. • Develop data collection forms in partnership with CHSC, including Quarterly Reports, client visit forms, Client Surveys, and additional tools, as needed • Design, update, and test data collection system(s) to reflect any changes to queries and reports • Coordinate and troubleshoot monthly, quarterly, and/or annual data reporting from Third Party Electronic Health Record system(s) • Train SHC sites and provide site-specific technical assistance on how to complete data collection forms, record and enter data, and/or generate data reports • Update and distribute brief training guides to all staff • Conduct at least two (2) group trainings for SHC Lead Agency staff annually • Partner with SHC Lead Agency staff to monitor and analyze utilization of SHC services • Collaborate with SHC Lead Agency on establishing and monitoring annual Quality Improvement Goals • Provide technical assistance per SHC Lead Agency per year by responding to telephone and email inquiries • Coordinate with Third Party vendor(s) related to Electronic Health Records data collection fields, reports, data cleaning, and data use for evaluation purposes • If data is available, analyze matched SHC client level data and school academic data from school district(s) Data Management: Ensure data is entered correctly and clean data as needed. • Provide site-level monthly data reports to SHC directors and CHSC staff • Promptly respond to external funder and CHSC requests for data (approximately eight [8] hours per month) within the scope of the existing evaluation design • In collaboration with school district contacts and West Ed (administrator of CHKS survey assist with administration of bi-annual CHKS SHC Custom Module in Alameda, Berkeley, Emery, Hayward, New Haven, Oakland, San Leandro, and/or San Lorenzo Unified School Districts • Work with West Ed to set up and format CHKS SHC Custom Module for administration
