1.1. Contractor shall operate a Short-Doyle Medi-Cal certified program to provide Full-Service Partnership (FSP) Strengths-Based Case Management (SBCM) for Older Adults (OA) ages 60 and over, which shall include recovery-oriented strengths-based clinical case management services to persons with serious mental illness (SMI). 1.2. Contractor shall provide strengths-based case management, rehabilitation and mental health services with a rehabilitation and recovery focus to adults who meet eligibility criteria. Bio-Psycho-Social Rehabilitation (BPSR) principles, as described below, shall be evident and operationalized in Contractor’s policies, program design, and practice. All services shall be provided in accordance with County Behavioral Health Services policy in the areas of dual diagnosis/co-occurring disorders (COD), older adult services, and cultural competence. 1.3. Services shall be coordinated and integrated with County of San Diego, Health and Human Service Agency (HHSA) Behavioral Health Services (BHS), Fee For Services (FFS) and other behavioral health providers, HHSA Aging & Independence Services (AIS), HHSA Child and Family Well-Being, HHSA Self-Sufficiency Services, Healthy San Diego providers, and other physical health providers, regional collaboratives, housing authorities, community resources and other organizations and groups serving mental health clients. 1.4. As an organizational provider agency, Contractor shall provide administrative and direct program services to County’s Medi-Cal clients [as defined in Title 9, Division 1, Chapter 11 of the California Code of Regulations].
