Specifications include, but are not limited to: The County of San Diego is seeking proposals from firms interested in providing Black Infant Health Program Services ; Contractor shall implement the San Diego County Black Infant Health program and provide outreach and recruitment activities in targeted zip codes in San Diego County. Case management and group session activities and case management only activities shall focus on improving birth outcomes for African-American women of childbearing age, and the health and well-being of their infants. Contractor shall enroll and provide follow-up services to AfricanAmerican women at risk for poor birth outcomes. Activities shall include participant and community education and prevention on the importance of healthy lifestyle choices, early access and utilization of prenatal, postpartum, and wellchild care.
