A. Services and all requirements as stated below and as required by law shall be provided by the Contractor, at the request of the Contract Manager, or designee. B. Contractor must meet the requirements and have the ability to adequately provide services to the Home’s Residents. Contractor may use Providers to perform services who meet the requirements specified in this Agreement. Contractor must provide the State with proof of any required training, licenses, permits, certifications and any other documents required by law including documents for each Provider performing services under this Agreement. C. The Contractor shall provide the following services: 1. ACTIVITY TRANSPORTATION: a. Contractor shall provide scheduled wheelchair and ambulatory transport of residents and CalVet support staff to a variety of recreational and cultural activities. Activities may be scheduled up to seven (7) days per week. Hours will vary depending on location and duration of activities; however, activities are typically, but not limited to, six (6) hours in length and occur between the hours of 6 AM and 1 AM. Scheduled activities may include any configuration of staff and residents (ambulatory and non-ambulatory), as well as necessary mobility devices such as wheelchairs, scooters, and walkers. Contractor shall be informed of the configuration of passengers and devices at the time of scheduling the activity; however, the Home reserves the right to modify the configuration at the time of loading as long as it does not exceed the capacity of the original request. b. Home shall make every attempt to notify Contractor a minimum of seven (7) calendar days in advance of scheduled activities, and Contractor shall call the Home to confirm availability of transportation within twenty-four (24) hours of request to schedule the event. Contractor shall respond to short notice requests of less than seven (7) calendar days within twelve (12) hours. The Home reserves the right to utilize internal resources for transportation to activities as available. c. Contractor shall arrive no later than thirty (30) minutes prior to scheduled departure time to facilitate loading of residents, staff, wheelchairs, and scooters. D. Contractor and Provider shall ensure that all ordered services have prior authorization in accordance with the Home and are mutually agreed upon between the Contract Manager, or designee, and the Contractor. E. Contractor/Provider shall ensure resident medical record information is maintained as “CONFIDENTIAL” and shall be held in trust and confidence. Information shall be used only for the purposes contemplated under this Agreement. F. Reports 1. Contractor shall provide a written report to the Contract Manager, or designee, upon completion of the transport, Invoices will only be paid if accompanied by a clearly legible report attached to invoices and approved by the Contract Manager or designee. 2. The reports shall be submitted in electronic format and sent via electronic mail as requested by CalVet, and indicate the following information: a. Pickup/Drop-Off Date, Time, Address b. Type of Transport
