1. The Contractor must provide the CV STAR Program Director or designee with verifiable proof of any required registrations, licenses, permits, certifications, and any other required documents noted herein, for any staff providing services under this agreement. Services will be performed only by the staff meeting the requirements outlined in this Agreement. Services cannot start until the license verification process and approval is completed by the CV STAR Program Director or designee. 2. The Contractor must possess and maintain a current, valid business license throughout the term of this Agreement. 3. All cleaning supplies used on the premises must be preapproved by the CV STAR Program Director or designee and are subject to ongoing review. The Contractor must provide a Material Safety Data Sheet (MSDS) or Safety Data Sheet (SDS) for chemicals used. 4. Provide weekly residential cleaning services for: restrooms, client rooms, storage areas, kitchens, waste areas, linen rooms, breakrooms, shower rooms, treatment rooms, hallways, training room, and offices. Contractor shall be able to move all furniture to allow a thorough cleaning of areas under and behind furniture. 5. Contractor shall provide all labor, cleaning implements, and cleaning supplies necessary to provide residential cleaning services for CV STAR home locations.
