• Reviewing all applications received by OCTA, including applications received electronically and by mail • Determining applicant eligibility based on the criteria provided by OCTA - Ensure application is completed in its entirety and any required documentation is provided, e.g. copy of various ID cards (Medicare card, Braille Institute ID card, Service-Connected Veteran ID card, DMV license/senior ID, other government issued ID card), proof of current semester enrollment and unit amount - Verify doctor license in the California Medical License Board website Search - DCA when necessary - Make follow-up calls to the certifying professional who certified the application when necessary for clarification • Inputting required data into the RFID database (AFM, OCTA will provide access) - Applicant name, street address, city, state, zip code, telephone number, date of birth - Two (2) digit code to indicate type of RFID (disabled, senior, mobility, ACCESS) - Two (2) digit code indicating eligibility code - Approval or reapply date - Notes if needed • Emailing approval, denial, or clarification of requirements • Scanning and attaching application and attachments • Generating mailing labels for approval notifications, affixing to notice as preparation for mailing (OCTA will provide blank notice postcard, see attached) • Preparing denial letters as necessary as required for mailing
