A. Field Services/Sample Collection Civil service staff will be performing the sampling primarily. When requested by the Institution Contract Liaison or designee, the Contractor shall collect samples, record on chain of custody form, and transport or mail to a qualified laboratory. The Contractor shall document time spent collecting samples on the Exhibit A-1, Service Report to be signed by the Institution Contract Liaison or designee and included with the invoice for services. B. Courier Services/Sample Transport When requested by the Institution Contract Liaison or designee, the Contractor shall provide shipping materials for transporting samples to CDCR institutions and mailed to a qualified laboratory. In some cases, the Contractor may be required to provide courier services that shall be scheduled to ensure delivery to the laboratory within the applicable “hold time”. C. Emergency Services The Contractor shall provide a telephone contact list for emergency services outside of normal business hours. The Contractor shall respond to emergency calls 24 hours a day, 7 days a week. Within twelve (12) hours of telephone notification by the Institution Contract Liaison or designee, the Contractor shall pick up the sample and initiate analysis immediately upon receipt of the sample at the laboratory. The Contractor shall provide verbal results within 12 hours of test/analysis completion. D. Analytical Services The Contractor shall analyze samples in accordance with the method identified on the chain of custody and represented in Exhibit B-1, Rate Sheet. The CDCR reserves the right to request samples not listed in Exhibit B-1, Rate Sheet. These tests shall be invoiced at the rate identified on the published price list and/or supplier’s invoice, which shall be itemized and include a 40% discount. All analysis will be conducted following standard turn-around times for the method identified, unless otherwise specified on the Chain of Custody. Sample analysis will begin the same business day of receipt, and determinations shall be provided in accordance with applicable procedures, rules, and regulations.
