Specifications include, but are not limited to: The successful Proposer shall furnish all personnel, equipment, and services necessary to provide Disaster Case Management (DCM) services for the purpose of assisting individuals and households impacted by a declared disaster in El Dorado County. In collaboration with County staff, the DCM program shall provide support, referrals, and assistance for survivors of a “Disaster” under broad supervision. Disaster Case Managers shall work directly with clients coordinating services for those who are “Disaster” survivors in the implementation and success of their case plan performing services including the following at a minimum: a) Carries a caseload of individuals impacted by a “Disaster” b) Determines the specific needs of those seeking assistance c) Assesses the level of need through home/phone/video visits in areas such as short-term housing, long-term housing, replacement of belongings, and mental health services d) Develops an individualized Disaster Recovery Plan with each client, monitors and follows-up regularly through home/phone/video visits to reassess client status e) Provides case management services for clients and assists clients in navigating requirements for rebuilding and/or accessing services f) Updates and maintains client files and records, records and documents services provided in a shared, cloud-based DCM Database system g) Participates in meetings through in-person/phone/video visits to review client progress, concerns, and recommendations h) Recognizes cultural and linguistic differences of client populations; provides service delivery to appropriate public, private or community services or resources, and provides language interpretation services and reasonable accommodations as needed i) Supports the relationship between County and the constituent population by demonstrating courteous and cooperative behavior when interfacing with clients, visitors, and other employees j) Completes and submits monthly reports (e.g., level of need cases broken out to include how many households & individuals in each level, how many hours of service were provided per individual, how many total households and total individuals, etc.) k) Adheres to the standards of the Privacy Act of 1974, {U.S.C. § 552A} as amended; maintains and enforces all aspects of confidentiality of client information. The successful Proposer shall perform all services in accordance with that HIPAA Business Associate Agreement which is attached hereto as Exhibit D to Attachment A.
