Specifications include, but are not limited to: The services include the following: 1. Intake & Referrals: Selected program will receive referrals from the Department of Public Health or its authorized Behavioral Health Services (BHS) providers and Human Service Agency’s (HSA) CAAP staff. The selected Proposer will maintain communications with referral sources of service availability and coordinate with clients for intake appointments. 2. Money Management Services: Each client will be assigned to a rep payee money manager who will serve as the client’s representative payee. The rep payee money manager involves the client in their financial service plan and reviews the client’s economic status. The rep payee money manager works with the primary BHS case manager to help keep the client stably housed with assistance with budgeting and teaching financial responsibility. 3. Representative Payee Services: Coordinate all aspects of financial arrangements, especially with depositing monthly income, budgeting, reviewing budget with clients, and issuing payments to landlords, utilities, etc. 4. Use of Department Electronic Health Record System (EHR): Provider will enter client’s demographic data in the Department EHR. Maintain appropriate human and technological resources to manage the accounting. DPH will provide access to EHR which might change during the contract period.
