On an as-needed basis, the Contractor agrees to provide pre-employment examinations required for determining if an applicant is physically qualified for a specific job at the Department of Developmental Services’ (DDS) Southern Stabilization, Training, Assistance, and Reintegration (South STAR) Community Crisis homes in Costa Mesa, CA. South STAR homes are five-bed community crisis homes for clients with intellectual and developmental disabilities (IDD) and mental health conditions requiring specialized supports and services. Additional information about DDS’ STAR program can be found here: DDS STAR homes. Applicants will be potential employees of: South STAR 1: 1 Mark Ln. Costa Mesa, CA 92626 South STAR 2: 2 Mark Ln. Costa Mesa, CA 92626 II. License Requirements and Qualifications Contractor and Contractor’s employee(s) who perform the physical examinations required in this agreement must be: A. A physician and surgeon licensed by the Medical Board of California (MBC) B. Licensed by the MBC, Physician Assistant Committee, as a physician assistant (PA) C. Licensed as a nurse practitioner by the California Board of Registered Nursing (BRN) III. Contractor’s Facility Contractor’s facility must be within a 30-mile radius from DDS South STAR homes, with all required testing equipment at one location.
