A. Duties/Responsibilities shall include, but are not limited to: 1. Dentist: a. Perform dental consultation, assessments, and procedures for residents as requested by the CP&S or designee. b. Perform surgical dental procedures for residents using local anesthesia and/or conscious sedation only. The procedures may include x-rays, restorations, tissue biopsies, extractions, and minor oral surgery and prosthetic treatment for pre-prosthetic placement of dentures. Excluding implants, cosmetic dentistry, or complicated appliances without prior approval. c. Examine, diagnose, and prescribe medications and instructions in a manner to assure optimum oral health for residents. d. Order lab tests and request consultation from other disciplines relative to the resident’s dental care. e. Prepare and maintain clinical records for each resident in which services are provided. These records shall be maintained in accordance with Title 22, Sections 53284 and 53861, and reflect all aspects of patient care, including ancillary services. f. Provide monitoring and oversight of dental instrument sterilization. If using an off-site laboratory, confirmation of sterilization shall be submitted to the CP&S to ensure verified sterilization of all instruments prior to use. g. Provide clinical supervision to the Dental Hygienist and Dental Assistant. 2. Dental Hygienist: a. Under the clinical direction of the licensed Dentist, perform professional dental hygiene care to include cleaning teeth, and assessing and evaluating patients’ dental hygiene. b. Perform routine dental procedures to include removal of plaque and calculus, scaling and root planning, polishing, application of fluoride, and other preventative and antimicrobial agents. c. Administer local anesthetic when necessary. d. Take necessary dental radiographs to aid in the assessment of dental health. e. Provide oral hygiene strategies, instruction, and nutritional counseling to residents. f. Maintain instruments and ensure they are sterile and in good working condition. 3. Dental Assistant: a. Under the clinical direction of the licensed Dentist, perform supportive dental procedures. b. Duties shall include, but not be limited to, infection control, radiographic assistance, and other clinical duties. c. Provide administrative support to Dentist and Dental Hygienist. d. Maintain instruments and ensure they are sterile and in good working condition. B. Contractor and Provider shall ensure that all ordered services have prior authorization in accordance with the Home and are mutually agreed upon between the CP&S, or designee, and the Contractor. C. Provider shall obtain the approval and authorization from the CP&S prior to referring a resident to other health care/medical vendors/contractors. Referral shall be made only to those health care/medical vendors/contractors set forth on the Home’s health care/medical vendor/contract list. D. Prior Authorization Documentation shall include, but is not limited to, the following: 1. Veterans Home rehabilitation referral process 2. Prior Authorization Number 3. Physician’s Consultant Form 4. Resident progress notes/summary pertinent to reason for referral 5. Laboratory and X-Rays (if applicable)
