Contractor shall provide off-site services as mutually agreeable by the Contractor and Contract Manager. Services generally occur during Monday through Friday, 8:00 AM and 4:00 PM, excluding weekends and State holidays. Contractor will respond to the Contract Manager, or designee within 24 hours whenever dates and times for services are established and changed. 3. Provide dental health care services to Residents in accordance with Medicare and Medi-Cal standards and requirements as set forth in Title XVII and Title XIX of the Federal Social Security Act, Title 22, Division 3, of the California Code of Regulations (CCR) and under the general direction of the Contract Manager 4. Contractor shall provide services to residents of the Home within twenty-four (24) hours notification by CalVet or as mutually agreed upon between the Home Contract Manager or designee and Contractor. 5. Provide consultation and assessment, treatment plan, and monitoring of response to therapy. 6. Maintain all dental records in the required manner in accordance with good medical and professional practice(s), which permits effective internal professional review and external medical audit processes facilitating an adequate follow-up treatment and all supporting documentation required by Federal and State Medicare and Medi-Cal laws, regulations, rules, and policies. All resident dental treatment records shall be maintained with summary provided for the Home’s Medical Record. Medical records shall be maintained in accordance with state and federal regulations, Title 22, CCR, Sections 53284 and 53861, and reflect all aspects of patient care, including ancillary services.
