The purpose of this Request for Proposals (RFP) is to solicit proposals from qualified occupational health and medical services providers with multiple facilities, physicians, and other medical service personnel within the agency's four-county (Los Angeles, Orange, Riverside, and San Bernardino) area of jurisdiction. To minimize employee travel time in the event of a "first aid" injury, the medical services provider must also have at least one full-service medical services facility within a 10-mile radius of South Coast AQMD's headquarters in Diamond Bar, California. Contractor will perform medical examinations required by the California Division of Occupational Safety and Health (Cal/OSHA) regulations regarding respiratory protection, medical surveillance, and potential employee exposure to toxic substances. Contractor will also provide medical evaluation and treatment to South Coast AQMD employees for pre-employment examinations, first aid for minor injuries, return-to-work evaluations following extended leaves of absence, fitness-for-duty examinations, and other medical examinations, as requested by South Coast AQMD.
