Specifications include, but are not limited to: The City of West Sacramento is soliciting proposals from qualified firms or individuals ("Proposer") to provide Occupational Health Services. 1 Pre-Employment Physicals Provider may be required to provide post-offer pre-employment medical examinations to ensure applicants are medically qualified for performing within the parameter of their classifications in accordance with the Occu-Med Medical Evaluation System (see Exhibit C). The City anticipates approximately 50 pre-employment physicals and approximately 150 drug screens will be performed annually. 2. Fitness for Duty Exams Provider may be required to conduct fitness for duty exams as requested by the City and provide reports as to employee work status based on that exam. 3. Blood-borne Pathogen Program Provider may be required to provide vaccinations and treatment in accordance with the City’s Bloodborne Pathogen Program. 4. Drug and Alcohol Abuse Program Provider may be required to provide substance abuse testing and reporting in accordance with the City’s Drug and Alcohol Abuse Policy which includes pre-employment testing, DMV physical examinations for commercial drivers; random drug and alcohol testing; last chance agreement follow-up testing; postaccident testing; and reasonable suspicion testing. All testing and reporting will comply with the requirements of the Omnibus Transportation Testing Act (OTTA) of 1991.
