Specifications include, but are not limited to: Contractor shall provide prevention and early intervention mental health services to older adults (age 60 and older) that are homebound and/or social isolated. Homebound older adult prevention and early intervention (PEI) services is a preventive and short-term early intervention program to address mental health symptoms and conditions that lead to anxiety, stress, and depression among older adults/seniors. Contractor shall ensure that services meet the needs of multi-cultural communities. The program will use the PEARLS (Program to Encourage Active Rewarding Lives for Seniors) model as an evidence-based approach to reducing social isolation, reduced risk factors for depression and other mental health conditions negatively impacting the wellness and health of homebound and social isolated seniors.
