Specifications include, but are not limited to: Contractor shall implement a multicultural and linguistically appropriate Prevention and Early Intervention (PEI) service that provides outreach, education, advocacy, peer mentoring support and transportation coordination services to unserved and underserved Latino, African American, Asian, Pacific Islander, Filipino, East African and Middle Eastern seniors. Contractor PEI services shall enable timely identification of mental health issues and the prevention of mental health issues, reduce inappropriate utilization of hospital emergency rooms (ER) and inpatient hospital admissions, enhance timely access and engagement to need mental and physical health services, enhanced resource utilization, service capacity and improve quality of care by providing culturally and linguistically effective services to seniors. Contractor shall incorporate the role of a Promotora/Community Health Worker as a liaison between senior communities and health, human and social service organizations to bring information to different multi-cultural senior communities. The Promotora/Community Health Worker shall serve in the role and function as an advocate, educator, mentor, outreach worker, role model, cultural broker, and/or translator to senior participants. In addition, the Contractor will incorporate the Office of Refugee Resettlement (ORR) guided program to link East African and Middle Eastern refugees, asylees, or Iraqi and Afghan immigrants, age 60 years and older, who possess Special Immigrant Visas (SIVs) to access social and health services, resources, and to assess and refer participants in need of citizenship classes.
