Specifications include, but are not limited to: Contractor shall operate a Short-Doyle Medi-Cal certified program to provide Institutional Case Management (ICM) program component for Older Adults (OA) ages 60 and over. The ICM component shall serve clients who are on Public Conservatorship and reside in a skilled nursing facility (SNF) or other County-identified long-term care institution (LTC). Services shall be coordinated and integrated with County of San Diego, Health and Human Service Agency (HHSA) Behavioral Health Services (BHS), Fee For Services (FFS) and other behavioral health providers, HHSA Aging & Independence Services (AIS), HHSA Child and Family Well-Being, HHSA Self-Sufficiency Services, Healthy San Diego providers, and other physical health providers, regional collaboratives, housing authorities, community resources and other organizations and groups serving mental health clients.
