Behavioral Health Services Provided − Submit a project proposal explaining how MHBG funds will be used to provide services. ο Provide outpatient mental health counseling services to adults and children with SMI and SED diagnosed with a behavioral, mental, or emotional condition as outlined by the Diagnostic and Statistical Manual of Mental Disorders (DSM) via a licensed mental health clinician. Services include individual and family counseling, crisis counseling, support groups, community education and activities aimed at enhancing and promoting Native American culture. ο Provide crisis support, referral services, community mental health interventions, SUD services and prevention activities. ο Provide outreach to 6 -12 graders for after-school pro-social, substance use prevention and harm reduction activities. ο Offer planned community outreach, specifically Native American culturally based psycho-education group services for children and adults. Data Gathering, Outcomes, Reports – − The provider agrees to provide monthly data reports on client demographics and session frequency to the County − Provider will agree to submit a monthly performance report to HHSA. − The provider agrees to submit monthly fiscal reports/invoices to the County for the purpose of reimbursement, no later than the 10th of the month, for the previous month (i.e. submit October monthly fiscal reports by November 10). − The provider and the County agree that the performance of work will be monitored during the life of the agreement as follows: ο Formal meetings between the provider and County representatives will be held monthly to monitor progress, discuss information in interim reports, and ensure compliance. ο Prior to payment, the County will review invoices and monthly reports for compliance with statement of work.
