1. Contractor agrees to provide Medical/Physical Examination and Hepatitis A (HAV) and/or Hepatitis B (HBV) vaccination Services to the California Department of Transportation (Caltrans), as described herein: Contractor shall employ a physician who shall be licensed in accordance with the laws of the State of California and who shall possess a Medical Board of California Certified Physician’s license. Contractor shall provide all necessary personnel, labor, equipment, material, and testing facilities to perform the following examinations in Kern County: A. Department of Motor Vehicles (DMV-DOT) Renewal Examinations B. Respiratory Compliance Examination C. Audiometric Examination (non-preemployment) D. Pre-Employment Medical and Audiometric Examination E. Administer Hepatitis A and/or Hepatitis B vaccinations 2. Subcontracting is not permitted under this Agreement. All references to subcontracting or subcontractors as found herein are not applicable to this Agreement. 3. The services shall be performed at the Contractor’s facility, which must be located within an estimated 15 miles from the Caltrans District 6 South Region Maintenance facility, located at 1226 Olive Drive, Bakersfield, CA 93308. 4.
