Work to be Performed Medical Evaluations • Initial (baseline) and periodic (every 2-3 years based on age) evaluations. • Online OSHA Respirator Medical Evaluation Questionnaire. • In-person clinical evaluations not to exceed two hours per visit. Services to Be Provided • OSHA questionnaire review • Physical examination • Spirometry (when needed) • Laboratory tests (subcontracted to Quest Diagnostics) • Lipid panel • Vision test • Job hazard assessment • Physical evaluation and respirator clearance determination • Follow-up testing (as required) 1. PHYSICAL EXAM AND HISTORY INTAKE: a. Height/Weight The employee’s height and weight using a balance beam scale and stadiometer will be determined and recorded. Height and weight parameters will be used to calculate various other physiological and anatomic values. Height will be taken without shoes. Weight will be taken with indoor clothing and without shoes. b. Blood Pressure Blood pressures will be measured using sphygmomanometers or automated blood pressure monitor. The employee will be in a sitting position during the reading. If blood pressure results exceed either systolic (140) or diastolic (90) reference range, the test will be repeated after the employee has been at rest for 13 10 minutes. If the employee has been diagnosed as hypertensive, all medications that the employee is using to control the hypertension will be recorded. c. Pulse A radial pulse will be measured manually or by automated pulse monitor. The pulse rate and rhythm will be recorded. d. General Appearance and Development It will be recorded if the employee is overweight. Any posture defects, perceptible limp, tremor, or other defects that might be caused by alcoholism, thyroid intoxication, or other illnesses will be noted.
