The Contractor shall provide all necessary materials, labor, equipment, tools, permits, and personnel to conduct medical evaluations in compliance with the California Division of Occupational Safety and Health Administration’s (Cal/OSHA) Respiratory Protection Program on an ongoing basis throughout the term of the contract. The Contractor shall provide a minimum of one (1) fully equipped Mobile Medical Van tailored and configured to provide on-site medical services. These examinations shall be provided at regular intervals throughout their careers. The types of examinations may include, but are not limited to the following: (1) Pre-employment Medical (PEM) Examination – The medical examination shall be recorded using the Physician’s Report portion of the STD. 610 Health Questionnaire form (State Personnel Board requirements) (2) The Department of Transportation (DOT) Examination for Commercial Driver's Licenses (CDLs) - The DOT Medical Examination Report (MCSA-5875) shall be used to document the medical examination, and the DOT Medical Examiner’s Certificate (MCSA-5867) shall be completed by the examiner. (3) Respiratory Protection Program (RPP) Examination – The medical examination shall be recorded using the Physician’s Report portion of the STD. 610 Health Questionnaire form.
