A. Case Management o Evaluate client readiness to transition to permanent supportive housing. o Work with clients to apply for and secure resources to ensure continued connection to resources and services, such as identification, income such as SSDI, SSI, CalWorks or unemployment benefits, EBT/SNAP, Housing Choice Vouchers (Section 8), Violence Against Women Act (VAWA), Veteran Affairs Supportive Housing (VASH) or other types of vouchers. o Ensure clients have all necessary documentation and meet all applicable deadlines for resource program applications and renewals. o All clients to be reported in the Homeless Management Information System (HMIS) which includes their names, demographic information, medical history and other pertinent information. o The service provider will be required to enter this information and provide monthly updates and referrals to the Coordinated Entry System (CES) as well as regularly attend and present clients at Pathways to Home Case Conferencing meetings. B. Provision of Interim Shelter o Provide non-congregate interim shelter services for up to 30 clients for a minimum of six months to a year upon execution of agreement. o The Motel 6 located at 2145 E. Harbor Blvd. is the current location of the interim shelter. Each household has their own room. o The program allows 2 pets per room and currently has several dogs and cats.
