A Warm Handoff Referral Process that district authorized users can access to submit referrals for students, staff and families in need of mental health or substance use treatment providers. 3. Digital parental/guardian/caregiver consent process integrated with Warm Handoff. System must have digital signature capabilities that are compliant with applicable state and federal laws (e.g. valid and legal signature requirements). 4. Referrals to providers for advanced pathways of care to include: outpatient, intensive outpatient, substance use clinics, psychiatry, residential, inpatient, 5150 wrap around school, medical detox, rehabilitation, hospitalization, stabilization, crisis intervention 5. Unlimited licenses for designated staff members to be registered users of the platform and tools. 6. A FERPA compliant dashboard that enables real time tracking and timeline of communications include, available matched providers and confirmation of scheduled appointments. 7. 24/7/365 multilingual care coordination support via text, video chat, phone or email that matches available providers, navigates all insurance and payment options, makes appointments, and follows up the day after and two weeks into care to ensure satisfaction with the provider. 8. Contractor shall provide assistance with insurance navigation of all private HMO/PPO insurance plans and sliding scale, low-fee or free services for families without insurance. Support accessing mental health services through public insurance processes. 9. An anonymous database, tool where students, staff and families can search for verified community providers who match their needs including mental health concern(s), wait, availability, insurance, or financial needs, language needs or other preferences for care such as proximity, gender of therapist, and/or LGBTQ+ allied services. 10. A database of verified local and statewide mental health providers who offer in-person and telehealth services to address all pathways of care for mental health and substance use needs.
