• Objective: To assist San Leandro residents at or below 30% Area Median Income (AMI) with rental subsidies that reduce the likelihood of eviction and housing instability. • Target Population: Extremely low-income households in San Leandro, earning 30% of Area Median Income (AMI) or below, as defined by the 2024 HUD Income Limits. Target population must meet eligibility requirements as outlined in Attachment A-1. • Key Services: • Eligibility Determination: Screen applicants based on income level, housing status, and eviction risk. • Rental Subsidy Distribution: Provide a monthly subsidy in the range of $500– $800 per household to cover a portion of rent. Funding will be provided for up to 18 months, or until program funds are exhausted. • Case Management: Offer ongoing support to ensure long-term housing stability, including financial counseling, budgeting assistance, and referrals to supportive services. Case Management can be provided directly or through subcontracted provider. • Data Collection and Reporting: Track metrics on housing retention, subsidy usage, and financial improvement to evaluate the program’s impact.
