Grantees will provide one or more of the HDG program models listed below: District-focused home-delivered grocery program District and single-room occupancy (SRO)-focused home-delivered grocery program Citywide home-delivered grocery program with centralized intake and waitlist services for DAS consumers Each program model will provide eligible consumers with a scheduled delivery of groceries to their homes. All grantees, regardless of the program model, are required to perform the following services and meet the following program requirements: Grantee will provide eligible consumers with a scheduled delivery of groceries to their homes. The standard frequency of deliveries is weekly. Contractors may propose an alternative scheduled delivery frequency; however, any such alternative schedule will be subject to approval by DAS OCP during contract negotiations. Grantee will leverage in-kind contributions, including but not limited to volunteers, to support their HDG program. The grantee is responsible for recruiting, training, and utilizing volunteers to assist with various aspects of the program, such as the preparation and delivery of groceries. Grantee will provide orientation and training to all new program staff, both paid and volunteer, to ensure they can perform their assigned responsibilities. All staff must complete food safety training before handling food, including packing for distribution and delivering to consumers. Additionally, the grantee will conduct in-service training for program staff at least twice per year, with one session focused on food safety to prevent foodborne illness and ensure food is safe for consumption. An annual in-service training schedule must be created and submitted to DAS at the start of each fiscal year. Grantee will ensure that sufficient qualified staff, both paid and/or volunteer, are in place to meet the program’s requirements. Staff must have the appropriate education, experience, and cultural competency to deliver quality services that meet the needs of consumers. Grantee will establish an agreement with the DAS Food Assistance Program (FAP) contractor, which provides weekly food distributions to designated Home-Delivered Grocery (HDG) contractors at no cost. These distributions are for consumers enrolled in DAS funded HDG program. The grantee will receive a selection of items from the USDA-defined food groups—fruits, vegetables, grains, protein foods, and dairy—in sufficient quantities and variety to allow individuals to prepare at least seven diverse meals a week. These meals should align with the healthy dietary patterns outlined in the most current version of the Dietary Guidelines for Americans (DGA), including any updates issued during the contract term. Grantee will have a designated drop site for receiving groceries from the DAS Food Assistance Program (FAP) contractor and preparing them for distribution to consumers. All drop sites must be reviewed and approved by the FAP contractor prior to use. Grantee will comply with the California Retail Food Code (CRFC) in all aspects of the home-delivered grocery program. The grantee must ensure that food received from DAS’s Food Assistance Program contractor is handled properly at the grantee’s drop site and distributed to consumers by volunteers or staff in accordance with federal, state, and local food safety regulations. This includes ensuring that food is safely stored, transported, and delivered to consumers’ homes. Grantee will establish and maintain a consumer enrollment process that includes eligibility verification, collection of required consumer data, and completion of a food security screening. The grantee will document consumer enrollment in their HDG program within SF DAS GetCare, ensuring that all collected data, including information from the food security screening, is accurately recorded. If necessary, the grantee will maintain a waitlist for services to ensure equitable access to resources, prioritizing the defined target populations. To facilitate this, the grantee will develop a prioritization ranking system to ensure that those with the greatest need receive assistance first. This ranking system must be approved by DAS. Grantee will implement a re-enrollment process every two years to ensure ongoing eligibility and accurate data collection. This process will include re-verification of consumer eligibility, an updated collection of required consumer data, and a new food security screening. The grantee will update consumer records in SF DAS GetCare to reflect any changes in status or information gathered during the re-enrollment process. Grantee will collaborate with other DAS-funded HDG contractors to minimize wait times for consumers on HDG waitlists. This collaboration may involve transferring or referring consumers to other contractors within the DAS-funded HDG network when appropriate. Grantee will conduct outreach activities to connect with the target population(s) and promote program services. Outreach is a core responsibility of the grantee and is essential for informing eligible consumers about the program.
