Specifications include, but are not limited to: The Napa County Health and Human Services Agency’s Mental Health Division (hereafter “County”) is expanding the current Mental Health Services Act (MHSA) Children’s Full Service Partnership (CFSP) to propose a caseload for Child Welfare Services and Juvenile Probation-involved youth with Serious Emotional Disturbance (SED) or Severe Mental Illness (SMI). The County has issued this Request for Proposals from qualified agencies/organizations to provide High Fidelity Wraparound Services for this population. Allocation of the available slots per month for Wraparound will minimally include Juvenile Probation and Child Welfare Services referrals. As the intent of this contract is to serve as many CWS and Probation involved children/youth as possible to prevent the need for higher level placement, the allocation of slots is fluid and may vary. These decisions will be determined on a case-by-case basis. QI staff shall: - Provide proof of certification and training in the use of CANS prior to completing any assessments. - Utilize CANS as part of youth’s assessment. - Upon receipt of referral, acknowledge receipt of referral via email. - Adhere to HIPAA guidelines in this process, including but not limited to necessary consents to share information. - Review referrals and documentation from initial assessments and previous court ordered evaluations as needed. - Conduct interviews with family and youth to obtain or clarify any information needed to complete functional assessments and formulate youth-specific short- and long-term mental and behavioral health goals. - Work in close collaboration with the pertinent clinicians, probation officers, child welfare workers, STRTP providers, and interdisciplinary placement committee (IPC) to gather information to formulate a determination.
