Specifications include, but are not limited to: The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) announces its intent to seek proposals from nonprofit organizations interested in providing Health Promotion programs to older adults and adults with disabilities. Health Promotion programs will cover two areas: 1) Self-Management of Chronic Health Conditions and 2) Physical Fitness and Fall Prevention. The purpose of the Self-Management of Chronic Health Conditions grant is to provide older adults with two evidence-based health promotion and disease prevention programs, approved by the California Department of Aging, focused on education and tools to help better manage chronic health conditions such as diabetes, heart disease, arthritis, and chronic pain. The purpose of the Physical Fitness and Fall Prevention grant is to provide older adults and adults with disabilities with health promotion and disease prevention programs focused on physical fitness and fall prevention.
