Specifications include, but are not limited to: Mental Health Supports or Trauma-Informed Responses Programs that support mental health well-being and/or programs that are provided in trauma informed and resiliency practices. Examples include mental health support groups, grief and bereavement support groups, implementing trauma informed care trainings; building awareness regarding inter-generational trauma; providing support to families on child social-emotional development and healthy communication; building emotional intelligence; mental health services; or addressing loneliness in youth or older adults. Parent and Child Health Programs that focus on health issues during pregnancy, childbirth, and postnatal care for family well-being. Examples include connecting pregnant individuals and families to culturally affirming programs that increase parental protective factors such as Black Infant Health and Fundamentals of Fatherhood; and providing culturally sensitive Midwife, Doula and Lactation Specialist Services for pregnant individuals or postpartum care. Chronic Disease Management and/or Health Education and Outreach Projects that promote early screening and prevention of health conditions, including chronic diseases, in culturally affirming local spaces (such as barber shops and salons, neighborhoods, and community events), build awareness of factors that affect health in communities and environmental health issues, connect residents to a health home and insurance benefits or health care rights. Examples include use of Promotora/Community Health Champion models, community fairs or trainings, case workers and referral programs, smoking cessation, or healthy eating and physical activity programs. Physical Activity and Engagement Activities that promote or provide increased physical activity engagement. Examples include after school programs for youth, nature exploration, group classes for older adults, and other culturally tailored activities (e.g., dance, sports, yoga).
