Project Description This project will involve coordinating a Harm Reduction and Ending the Epidemics (HIV/HCV/STI) Program that serves the most vulnerable populations in Marin to access harm reduction and low barrier health services. The harm reduction services should include distribution of harm reduction supplies such as syringes, Narcan, fentanyl test strips, and safer smoking/injecting supplies to prevent overdose and other harms of drug use. Ending the Epidemics activities include providing HIV/HCV/STI prevention, testing, linkage to care, and retention in care activities for individuals at high risk for HIV, HCV, and STIs. Outreach efforts should incorporate connecting individuals to community services and supports including basic health needs and substance use disordertreatment. The program must ensure that services are accessible to communities living in all regions of Marin County and with all insurance statuses. E. Intended Outcomes, Goals, or Objectives: Objectives include: • Distribute harm reduction supplies to priority populations. • Provide overdose prevention education. • Connect individuals to behavioral health and substance use disorder treatment. • Provide health education and awareness on HIV/HCV/STIs. • Conduct HIV/HCV/STI testing for priority populations. • Conduct navigation and linkage to care for people with a positive HIV/HCV/STI test. • Conduct care coordination and treatment (if applicable) among people with HIV/HCV/STIs. • Provide referrals to PrEP for eligible individuals. • Connect individuals to behavioral health and social services.
