A. Perform HCV testing navigation services for Orange County residents either enrolling or enrolled in MAT programs in Orange County, including: 1. Educating clients about HCV and conducting HCV screening, either via telephone or in person. 2. Assist as needed with navigation to an appropriate testing facility. B. Perform HCV Care Navigation Services for MAT clients who are Orange County residents and are diagnosed with HCV infection. including: 1. Assist clients in accessing a medical provider to assess and initiate treatment for HCV infection. 2. Provide client follow-up at least two (2) times during the course of client's treatment, via telephone or in person, to encourage completion of treatment** or identify barriers to treatment. 3. Provide client follow-up within one (1) month of treatment completion, via telephone or in person, to determine if treatment was completed and to educate the client that testing should be performed twelve (12) weeks after the completion of treatment to assess for cure. 4. Assist with navigating the client (via phone or in person) to a testing facility to check for HCV RNA to determine the results of treatment
