Contractors will provide Fall Hazard/ Site Safety Assessment and Mitigation Services to the District as described in this RFQ and the Request for Bid as Jobs are scheduled for performance. For the purpose of this RFQ, the term “Fall Hazard/ Site Safety Assessment and Mitigation Services” shall include, but not be limited to, fall hazard assessment, fall protection anchor installation, fall protection anchor inspection and testing, general site safety assessment, site safety mitigation design, engineering and installation. The Proposer must complete the Fall Hazard/ Site Safety Assessment and Mitigation Services questionnaire and schedule in Attachments E-1 and E-2, specifying the type of services available for which the Proposer will bid as jobs are scheduled. Service pricing is not required for the RFQ at this time. 1. Site safety hazard assessments include, but are not limited to: a. Visit and assess multiple sites b. Evaluate for required improvements to meet minimum OSHA standards c. Provide report with findings and recommendations 2. Site safety mitigation includes, but is not limited to the design, engineering and installation of the following: a. Fall protection anchor b. Improvements to existing fixed ladders c. Sloped surface access 3. Fall protection anchor annual inspection and testing
