A. The Contractor will schedule an ergonomic evaluation with the State Contract Monitor within three (3) days of the CDE Contract Monitor’s request by email or phone. The CDE Contract Monitor will provide the Contractor the CSDR’s employee’s contact information so that the ergonomic evaluation can be scheduled over the phone or by email. The ergonomic evaluation will be completed within seven (7) business days of the CDE Contract Monitor’s initial request. B. The Contractor will evaluate the CSDR employee’s assigned workstation. 1. The Contractor will interview the CSDR employee about how they perform their duties. The Contractor will gather physical and medical information from the CSDR employee which is pertinent only to the ergonomic evaluation and listen to any specific issues that the employee may have. 2. The Contractor will have the CSDR employee demonstrate how they perform normal work duties (i.e., standing, sitting, keying, etc.)
