Specifications include, but are not limited to: i. Comprehensive Office Ergonomic Evaluations: A comprehensive analysis includes: a) Analysis of the individual, job tasks and workstation. b) History of symptoms. c) Body mechanics. d) Workload and work practices. ii. Preventative Office Ergonomics Evaluations: The evaluation includes: a) Analysis of the individual, job tasks and workstation b) Body mechanics. c) Risk factor identification. d) Equipment/set up recommendations. e) Implementation of changes during the evaluation to the workstation when possible. f) Physical fitness. iii. Follow-Up Visits: Provide follow-up visit after the ergonomic evaluation, if needed. The vendor shall coordinate the logistics of the assessment appointment. If there are multiple assessments needed, the vendor will make its best effort to schedule the assessments back-to-back. iv. Provide a written report to the Department Liaison. The report must include: a) Date of assessment. b) Name of the individual receiving the assessment. c) Description of workstation configuration. d) Physical factors of the job. e) Job tasks and duties. f) Ergonomic risk factors and solutions...
