Specifications include, but are not limited to: The San Francisco Department of Public Health’s (DPH) Behavioral Health Services (BHS) and Whole Person Integrated Care (WPIC) sections (both entities of San Francisco Health Network (SFHN) have been providing critical client-based support services to vulnerable populations, including persons experiencing homelessness and/or who have complex medical conditions, mental health and/or substance use issues. DPH is releasing this Request for Grant Applications (RFGA) to solicit potential applicants who can provide expanded client-based peer support services to vulnerable populations. Peer and Paraprofessional Specialists will be embedded in various clinical teams to support critical citywide initiatives.
