Specifications include, but are not limited to: 1. Program Category #1: Peer Navigation and Outreach: a. Work with Gender Health SF to provide peer-based navigation and outreach services for unhoused TGD individuals who are seeking medical and/or social transition supports. Services may include but are not limited to: i. Helping clients understand the necessary steps for different types of gender-affirming care services (e.g. surgery referral process, peer support and navigation, peer education and support). ii. Accompanying clients to appointments. iii. Explaining and guiding clients through the process of identity document changes (e.g. legal name and gender marker changes.) iv. Linkages and referrals to promote engagement in care and community services. 2. Program Category #2: Behavioral Health Service Expansion: a. Work with the Department of Homelessness and Supportive Housing (HSH), Behavioral Health Services, and TGD-serving community-based organizations to provide short-term behavioral health services for unhoused transgender and/or gender-diverse (TGD) individuals in temporary supportive housing sites, such as navigation centers and shelters. Services may include, but are not limited to: i. Client triage and linkage to longer-term behavioral health services. ii. Counseling around treatment options for mental health and substance use needs. iii. Counseling around wellness practices and resources, and peer counseling. iv. Services should be primarily delivered in-person at sites where clients congregate and have telehealth available on as needed basis. 3. Program Category #3: Capacity Building and Training Development: a. Cultural Humility Trainings: Work with the SFDPH BHS Training Office and departmental subject matter experts to research, develop, and produce foundational and advanced cultural humility transgender health training for city staff and providers to deliver a better understanding of TGD individuals experiencing homelessness and enhance their capacity to effectively serve this population. Training can be delivered remotely and/or in-person, tailored to the audience's needs. These programs will adhere to current best practices in TGD behavioral health care, like the World Professional Association for Transgender Health (WPATH) Standards of Care. Topics may include, but are not limited to: i. Providing Respectful and Culturally Competent Care for TGD Individuals. 1) Using proper pronouns and preferred names. 2) Addressing misgendering instances. 3) Implementing gender-neutral language. 4) Transgender Health 101: Understanding. 5) Transgender People in Public Health. ii. Topics for Advanced Cultural Humility Training: 1) Medical and Social Transition Overview: Exploring the Transition Process and Addressing Needs of TGD Individuals Experiencing Homelessness in San Francisco. 2) Local Community Resources for TGD Individuals Experiencing Homelessness. 3) Delivering Culturally Responsive Care while Working with TGD Clients. b. Care for TGD Patients Experiencing Homelessness across the Lifespan Trainings: Work with SFDPH BHS Training Office and departmental subject matter experts to research, develop and produce specialized training programs for city clinicians, staff, and providers on clinical best practices for working with TGD clients experiencing homelessness across the lifespan. The training should be delivered remotely and/or in person and be tailored to the needs of the target audience. Trainings should be based on and conform to the most up-to-date best practices available in the field of TGD behavioral health care, such as the World Professional Association for Transgender Health (WPATH) Standards of Care. i. Possible Topics Include: 1) Evolving Health Needs of TGD Individuals Experiencing Homelessness: Addressing Health Concerns throughout the Lifespan. 2) Tailoring care for different TGD age groups and meeting the unique medical and psychosocial health needs of TGD Individuals. Through these training programs, we aim to foster understanding, competence, and effective care for TGD individuals experiencing homelessness while promoting the highest standards of behavioral health care in alignment with the most current best practices and Standards of Care.
