Specifications include, but are not limited to: provide as-needed Audiology Services for Residents of Laguna Honda Hospital & Rehabilitation Center or other Department location of the San Francisco Department of Public Health. Services will be provided weekly, as-needed on a schedule mutually agreed to by the Contractor and City. Services shall be performed between the hours of 8:00 AM and 5:00 PM. 1. Audiology evaluation including: a. Speech reception and discrimination testing b. Pure tone threshold assessments with complete audiogram 2. Audiometry Screening 3. Hearing aid evaluation, dispensing, and repair 4. Consulting with LHH staff physicians, nurses, and other members of the interdisciplinary team 5. Hearing therapy including: a. Counseling patients, family members and staff regarding use of hearing aids, assistive listening devices, and strategies for facilitating communication with hearing-impaired residents. b. Aural rehabilitation, if indicated. 6. Patient record keeping for clients served, including writing notes in the patients' medical record regarding findings, and recommendations for audiology care for clients served.
