Contractor shall perform or provide for the City the following administrative elements and services in conjunction with the Employee Assistance Program (EAP) throughout the term of Agreement: • A specially screened, qualified, and contracted full-range panel of professionals and institutions in agreed upon geographic areas. Contractor shall expand the panel as requested by City; however, providers are subject to Contractor credentialing criteria; • Toll-free access via phone, email, and/or text to Contractor by employees and eligible family members; • Employee/family benefit eligibility determination; • Provide customized reports to the City on a semi-annual and annual basis; • Maintain a live EAP intake line that operates for routine calls from 6 am to 5 pm Pacific time, and is available for emergencies 24 hours per day and staffed by licensed behavioral health providers; • Provide on-site counseling due to emergency issues within 24 hours when requested by the City; • Conduct phone interviews for assessment of the caller's problem area and a referral to an appropriate EAP resource; • Provide a minimum three (3) EAP counseling visits per member (employee and covered dependents) per 6-month benefit period. The EAP provider is reimbursed 100% by Contractor and the City has no financial liability. The counseling sessions are designed to specifically assess needs and to provide problem resolution services through counseling and psychological education materials; • Provision of telephone consultations to City's Management, relative to improved understanding of workplace and employee issues, as well as identification of resolution or referral options; • Provide counseling via texting, messaging, or real-time video session for employee and covered dependents in addition to traditional face-to-face counseling sessions; • Provide legal and financial consultation support; • Provide work -life balance coaching, including stress management, parenting, and time management. • Provide specific counseling for safety personnel and covered dependents in unique areas of the work performed by safety personnel; • Contractor case managers and supervisors shall have extensive training and experience in off-site and on-site debriefing, consultations, and direct counseling; • A network of experienced and credentialed professionals is available for larger scale critical incident responses; • Unlimited hours of Critical Incident Debriefing, Management Training, and ten (10) hours of educational trainings, or other on-site seminars. These services are included in the per employee per month fee; • Provide supervisor training programs, employee seminars, webinars; • Printed materials, audio/video recordings, and online access to childcare, eldercare, legal, relocation, financial counseling and other work life information and resources for eligible employees and family members; • Access to the Contractor website • Provide monthly emailed newsletters for employees • Provide report summarizing work life data in both email and hard copy formats. No individual data will be provided
