The Contractor will provide qualified medical personnel, equipment, and testing supplies necessary to perform three types of exams: DMV Physicals, audiometric testing, and pulmonary function testing. These tests will be in accordance with Title 8 of the General Industrial Safety Orders. a. The DMV Physicals will include the following steps, which the qualified medical personnel will use to complete U.S. Department of Transportation Form MCSA-5875 – Medical Examination Repot Form for each Physical administered: i. Review of the patient’s provided health history and comparison to their provided list of medications. ii. Testing and recording the patient’s pulse rate and rhythm, height, weight, and blood pressure. iii. Urinalysis testing and recording of numerical readings for specific gravity, protein, blood, and sugar. iv. Vision testing that measures uncorrected and corrected acuity and horizontal field of vision in each eye. v. Hearing testing in the form of either whisper testing or audiometric testing delivered to the patient via headphones. vi. A physical examination that checks the body systems listed on U.S. Department of Transportation Form MCSA-5875 for abnormalities. b. Audiometric testing appointments will measure the patient’s ability to hear sounds. Pure tones will be delivered to the patient via headphones to determine the minimum volume, or intensity, necessary for the patient to hear each tone. c.
