A. SEMI-ANNUAL MAINTENANCE SERVICES 1. The following equipment shall be serviced on a Semi-Annual basis and according to manufacturer instructions: A. Ritter by Midmark, M9 autoclave, B. Air Techniques, Air Compressor and Vacuum, and Acadia by Air Technique Amalgam Separator 2. Maintenance shall include as a minimum: A. Check exterior surfaces; B. Check for noise and leaks C. Clean, lubricate, and sterilize as needed; D. Flush lines as needed; E. Check all tubing for kinks or cracks; F. Check all gaskets, plugs, filters and trays; G. Calibration as needed; H. Repair and replace parts as needed to maintain equipment operating according to manufacturer’s recommendations. Parts in this section are items that are less than $20.00 and are usually found in the mechanic bag (such as screws, nuts, bolts, gaskets, 0-rings, etc…). Parts that are needed to be order shall be under Part F of the rate sheet and labor rate shall be defined in Part C (Regular Business Hour Service Visit) I. Documentation/tags certifying maintenance performed B. ANNUAL MAINTENANCE SERVICES 1. The following equipment shall be serviced on an Annual basis and according to manufacturer instructions: A. 2 (two) ADEC-Light –Radius Wall Mounts B. 2 (two) CAVITRON SCALER C. ARIBEX Portable, Hand-held X-Ray D. MIDMARK X-Ray Unit E. 2 (two) ADEC Dental Chairs F. 2 (two) ADEC Dental Chair DELIVERY Units 2. Maintenance shall include as a minimum: A. Check all exterior surfaces for wear and tear; B. Clean and disinfect chair and stool upholstery with recommended cleaning agent; C. Clean all values, tubes, lines; D. Calibrate equipment as needed; E. Repair and replace parts as needed to maintain equipment operating according to manufacturer’s recommendations. F. Documentation/tags certifying maintenance performed C. Contractor shall perform all maintenance, repair services, and performances assurance including testing and documentation necessary to meet all current requirements of the Department of Health Services, Title 22, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO). D. Contractor shall ensure Scheduled maintenance services and inspections including lubrication, cleaning, functional tests, leakage current tests, and adjustments to equipment are performed as specified. Equipment requiring calibration shall follow U.S. Bureau of Radiological Health Standards. E. Installation of New Equipment and Machine shall be according to manufacturer recommendation and requirement.
