A. The Contractor shall furnish all labor, including travel and per diem, materials, nonconsumable supplies, parking fees and bridge tolls, insurance, transportation, equipment and every other item of expense necessary to perform services. B. Contract price is for courier providing necessary one-way delivery service to fulfill the needs of all providers sending work to the CALPIA. See Exhibit A – County Listing for a listing of counties served primarily by the CALPIA dental lab. Counties are subject to change. C. The Contract covers a term from December 16, 2025, through December 15, 2027, for courier service for the dental laboratory listed below in Section 2. D. It is estimated that the lab will ship a total number of approximately 60 packages per day or 1,260 packages per month during normal working hours, Monday through Friday, 8:00 AM to 5:00 PM, except state holidays. The lab services approximately 40 providers. E. CALPIA does not guarantee any volume of deliveries under any contract. Packages include cast metal, partial frames, acrylic partials, dentures, and split/sport guards. Approximate package size: 90% of the packages weigh less than two (2) pounds; eight percent (8%) weigh less than five (5) pounds; and the remaining two percent (2%) weigh less than 10 pounds.
