a) Develop the internal infrastructure needed to establish a DSMES program targeting one or more of the SEDPM priority populations (i.e., Black, Latino, Asian/Pacific Islander, and/or low-income individuals in Los Angeles County). Effort may include identifying a DSMES curriculum, developing protocols, establishing documentation procedures, referral pathways with a partnering health system, developing recruitment materials, and training staff. b) Apply for DSMES accreditation/recognition from the American Diabetes Association or the Association of Diabetes Care and Education Specialists. c) Identify and recruit participants for 1-3 DSMES cohorts (approximately 5-10 individuals in each cohort). d) Develop a written summary of project progress, including lessons learned and recommendations for other organizations looking to establish a DSMES program.
