Scope of Work: A. Target Population. Clients comprise a diverse community that may include, but is not limited to, youth, families, uninsured individuals, under-insured individuals, previously incarcerated individuals, individuals with mental health challenges, and individuals who are chronic, active, or recovering substance users. Clients must have an individual/household income of less than 600% of the federal poverty guidelines, revised annually. Contractor must ensure eligibility of Program services for each potential Client by: a. verifying diagnosis of HIV/AIDS, b. verifying proof of San Francisco residency, c. verifying low-income status, and d. assist potential Client in gathering these necessary eligibility documents as-needed. C. Enrollment. Contractor must provide each eligible Client with the following: a. explanation of Client rights and responsibilities for participating in the Program, including outlining Client’s right and providing the necessary procedures for filing grievance(s). b. create an individual account for each Client. c. assist or complete application(s) to designate Contractor as Client’s representative payee which authorizes Client’s benefit(s) to be directly deposited into the Contractor’s managed account when possible...
