Specifications include, but are not limited to: A. Logo Development 1. The Trainer will subcontract with a graphic designer from the Deaf, Hard of Hearing, Blind, or Physically Disabled community. i. The Trainer will provide LACDMH with a copy of any IRS requirements for hiring subcontractors including Tax Identification Number and Workers Compensation Insurance. 2. The logo will be designed with the intent of branding the new Deaf, Hard of Hearing, Blind, and Physical Disabilities UsCC subcommittee to increase awareness and participation in the stakeholder process and promote this group amongst the training participants. 3. LACDMH to provide Trainer with names and contact information for possible graphic designers. 4. The graphic designer will develop a logo for the Deaf, Hard of Hearing, Blind, and Physical Disabilities UsCC subcommittee to be used in all materials related to this project. 5. This design will be vetted through the subcommittee and must be approved by the members. 6. The logo design should be completed within the first two months. B. Pre and Post-Test 1. The Trainer must develop and administer a pre and post-test at each of the trainings to gather information on the level of knowledge gained by the participants. 2. The pre and post-test must utilize a Likert Scale. 3. The pre and post-test must be submitted to LACDMH for pre-approval. 4. The Trainer must submit copies of all completed pre and post-tests to LACDMH.
