The Scope of Work for the Community Vitality and Family Reunification Initiative requires the Contractor to collaborate and coordinate with the City’s Code Enforcement, Rapid Response Teams, law enforcement agencies, and other local discharging institutions to develop and deploy a field-based, inter-agency program focused on outreach and diversion services. The primary objective is to facilitate the reunification of individuals experiencing homelessness or those at imminent risk of homelessness with their family, friends, or other support networks. This will be accomplished through a range of supportive services including case management, transportation, and costs associated with transportation, both within and outside the Bakersfield-Kern Continuum of Care (CoC). The Contractor will be responsible for engaging vulnerable populations in the field, offering timely and compassionate support to prevent homelessness from escalating, and ensuring individuals are connected to appropriate resources for long-term stability.
