HOUSING NAVIGATION SERVICES Provide a narrative outlining how your agency will implement the requested Housing Navigation Services. The narrative should include: • Program Overview: Detail the proposed program from start to finish, covering the entire process including referral, intake, ongoing services, and exit. Described how the program will collaborate with the City of Fresno’s Homeless Assistance Response Team (HART), the Fresno Police Department’s Homeless Task Force, the Coordinated Entry System, The Fresno County homeless services and other homeless service providers. Explain how these collaborations will work together to engage, serve, and stabilize the most vulnerable individuals experiencing homelessness, helping to prevent the harmful effects of homelessness, and to resolve encampments.
