The District is seeking proposals from qualified vendors to provide prevention and early intervention behavioral health services to the District’s students in grades TK-8 (“Services”). The selected vendor will ensure the delivery of accessible, effective, and evidence-based wellness coaching, in accordance with the California Department of Health Care Access and Information (“HCAI”) Certified Wellness Coach Employer Support Grant Program (“Program”), this RFP, the Services Agreement attached as Attachment C (the “Agreement”), and a California Student Data Privacy Agreement attached as Attachment D. The Services will include the selected vendor assisting the District to meet the District’s obligations related to the Grant Agreement by and between HCAI and the District attached to the Agreement as Exhibit A (the “Grant Agreement”). The Program provides wellness coaching services outside school hours, including evenings, weekends, and summer sessions, via secure video conferencing. The target population includes approximately 8 schools and 3,000 students who may opt in for support.
