Specifications include, but are not limited to: Contractor shall operate a certified outpatient behavioral health program and provide a full range of outpatient diagnostic and treatment services and intensive case management to stabilize high risk youth, adolescents, and young adults up to age 18, hereafter known as clients, who are full-scope Medi-Cal beneficiaries, low income or uninsured, who are leaving a psychiatric hospital and/or the Emergency Screening Unit (ESU) and in need of follow-up services to divert or prevent readmission to acute care services. Referrals from Psychiatric Emergency Response Team (PERT), Mobile Crisis Response Team (MCRT), Emergency Departments, and other County referrals may be considered for instances where youth are at risk for inpatient hospitalization without immediate support due to behavioral/emotional escalations. Services shall be short-term and transitional in nature until the child/youth is connected to behavioral health services within their community. Services shall adhere to Children’s System of Care (CSOC) principles and be community based, with services provided in the home, and/or office/clinic settings as well as leveraging telehealth as appropriate.
